Houston City College is committed to cultivating an environment free from inappropriate conduct of a sexual or gender-based nature including sex discrimination, sexual assault, sexual harassment, and sexual violence. Sex discrimination includes all forms of sexual and gender-based misconduct and violates an individual’s fundamental rights and personal dignity. Please complete this form to report an incident of sexual misconduct to the College and return it to the Title IX Coordinator:

Note: The College understands that some individuals filing complaints or who are otherwise involved in an incident may want to remain anonymous. The amount of detail provided will largely determine the College’s ability to respond to reports of sexual misconduct. In all cases, issues of confidentiality will be balanced against the College’s commitment to addressing incidents of sexual misconduct and providing a safe community.
